Parties eligibility and contacts
Taekit, LLC ("Taekit") administers the Taekit program, provides administrative, technology, and coordination services, and collects program payments. Clinical services are provided by Health Hub Physicians, Inc. ("HHP"), a Texas Medical Board certified nonprofit health organization, or another participating medical practice or healthcare organization responsible for your care (your "Clinical Provider"). Your program identifies the responsible Clinical Provider and participating dispensing pharmacy. An outside Clinical Provider may perform your intake, evaluation, prescribing, authorization, and follow-up without HHP's involvement. These terms describe Taekit's services and the clinical and pharmacy services included in your program. Each participating organization is responsible for the services it undertakes; naming an organization does not make it responsible for another organization's obligations.
Taekit does not practice medicine, prescribe, or dispense. Your treating licensed professional independently decides whether an evaluation, prescription, particular medicine, treatment, or referral is appropriate. The dispensing pharmacist independently reviews prescriptions and dispensing safety. Payment does not guarantee a prescription, every advertised medicine, later treatment authorization, or a particular clinical result.
This Texas program enrolls adults age 18 or older. Each patient needs an individual assessment and prescriptions; purchasing a kit does not authorize family members to use it. If buying for another person, use the gift-card option so the recipient can place an individual order and complete the clinical process. Employer-funded enrollment may use a separate benefit-activation process. Payment by a purchaser or sponsor does not give that person authority to consent to treatment for the patient. A legally authorized representative may act within the scope of verified authority.
Taekit administrative support. For administrative, website, order, payment, refund, or purchase-terms copy requests, contact support@taekitmedical.com. Taekit administrative support is provided by email only.
Clinical care and coordination. For clinical questions, authorization or replenishment requests, nursing communications, medical records, or clinical privacy matters, contact care@taekitmedical.com. The clinical nurse triage telephone number is supplied in your kit and is for clinical communications only. This centralized care team may coordinate with and route requests to the responsible Clinical Provider, including an outside participating practice. HHP's routine clinical service hours are Monday through Friday, 9 a.m.-5 p.m. Central Time. Messages may be left outside those hours, but HHP does not provide after-hours clinical coverage or a guaranteed response interval. Outside practices' availability and response arrangements may vary and will be explained for your program.
The initial standard fulfillment pharmacy is Medical Square Pharmacy, Texas pharmacy license #28832, 1401 Wirt Road, Suite E, Houston, Texas 77055, telephone 713-468-3236. A particular program may designate another participating pharmacy, whose identity and contact information will be provided in connection with your services. The designated pharmacy supplies and dispenses the medicines, assembles the kit, and arranges shipment directly to you.
Essential medication commitments
You agree to provide complete and accurate information during medical intake and subsequent care, including allergies, diagnoses, current medications and supplements, and pregnancy or breastfeeding status. You agree to contact the care team about changes in your medical history before starting kit medicine. You agree to continue medications prescribed by another provider unless a treating professional directs otherwise, and not to stop taking them merely to qualify for a kit. You agree to follow your provider's instructions and ask the clinician or pharmacist to resolve questions before use.
Before using any medicine or supplement supplied in the kit, you agree to contact the care team for an assessment and obtain patient-specific authorization from the treating medical provider. This program requirement includes products in your kit that are otherwise available over the counter. The initial prescription and pharmacy dispensing do not themselves authorize you to begin treatment for a later illness. A message left for the team, an order-status notification, or a refill request is not authorization.
Medical authorization covers the treatment course described by your treating provider, including the medicine, dose, timing, and duration. You do not need permission before every dose within that course. A new illness, repeat course, changed symptoms, or a material change in your health or other medicines requires renewed assessment and instructions before further use outside the authorized course.
Never share your prescription medicines. They are prescribed only for the person named on the label, even if someone else has similar symptoms.
Taekit medication cards and online resources are for general education. They do not independently provide a diagnosis, prescription, provider order or permission to start treatment. They do not replace a clinical assessment, your individual instructions, required pharmacy labeling, pharmacy-provided medication guides or pharmacist counseling.
If you are experiencing a medical emergency, call 911 or the local emergency number and obtain emergency care immediately. Do not wait for the care team to respond. The kit and this program are not emergency services. If you cannot reach the care team when you need care, obtain appropriate outside care instead of starting kit treatment on your own.
Medical evaluation and starting treatment
Your Clinical Provider may review your intake and relevant clinical information asynchronously, meaning at a different time from your submission, or use a live video, audio, in-person, or other legally sufficient encounter. An appropriately licensed registered nurse may contact you to reconcile information and triage symptoms. A physician, nurse practitioner, or physician assistant with appropriate authority makes prescribing and treatment decisions after adequate clinical evaluation. If you are currently ill, seek timely assessment through the care team or an appropriate outside provider; enrollment is not a promise of immediate treatment.
Your Clinical Provider will identify your treating clinician and credentials. At each clinical contact, provide your name, date of birth, current physical location, and a reachable telephone number, including when traveling. Care is available only where the assigned professional may lawfully provide it. If local law prevents that professional from treating you, the care team will direct you to seek an appropriately licensed provider where you are. There is no promise of care throughout the United States or internationally.
Telehealth limits the examination and testing that can be performed and may be affected by incomplete information, technical failures or privacy risks. Your clinician may require additional information, another consultation, outside examination or testing before prescribing or authorizing use. If a connection fails, the clinician may use another suitable method only if it permits an adequate and lawful evaluation.
Taekit does not operate an in-person clinical practice, laboratory, or emergency service. A participating Clinical Provider may offer in-person care or testing, but those services are included only if expressly stated in your program's disclosed benefits. You may seek outside care at any time. Alternatives include your primary care clinician, urgent care, a specialist, or an emergency department. Outside providers and testing services may charge separately. Your Clinical Provider will provide follow-up instructions and explain when outside evaluation or testing is needed.
Before treatment, the clinician will explain material risks, expected benefits, and reasonable alternatives appropriate to your circumstances and give you an opportunity to ask questions. You may decline a medicine or treatment, decline telehealth, or withdraw consent to future telehealth by telling the care team. The care team will route your decision to the responsible Clinical Provider. Withdrawal does not undo prior care or erase required records; your Clinical Provider will address appropriate transition needs. These terms do not replace informed clinical discussion or the applicable standard of care.
Telehealth and treatment consent
When enrolling as the patient, or acting as the patient's legally authorized representative, I consent to clinical evaluation and appropriate treatment by the Clinical Provider identified for my program. When telehealth is used, this includes nursing triage and reconciliation, asynchronous review by the medical provider, and a live encounter when appropriate, together with clinical data collection and lawful sharing needed for that care. I understand the limits, alternatives, and right to withdraw described under "Medical evaluation and starting treatment." I have an opportunity to ask questions before care and may decline recommended treatment. Purchasing a gift card or sponsoring another person's kit does not supply that person's treatment consent. This consent does not authorize promotional disclosures. Recording and AI use are described under "Privacy communications and recording."
When symptoms arise, you agree to contact the care team. The nurse will assess urgency, document your current information, and arrange review by a licensed medical provider when appropriate. The prescribing provider will decide whether a kit medicine is suitable now and document any authorized course. A nurse triage call by itself does not constitute independent permission to begin medication.
If you identify a primary care physician and consent to sharing the visit report, your Clinical Provider will send that physician a report within 72 hours after the telemedicine service, as applicable under Texas requirements. You may change or withdraw that consent for future reports.
Pharmacy individual contents and safe storage
The contents of the medication kit are subject to your clinician's individual assessment and the pharmacist's review. The clinician may omit or change a medicine that is inappropriate for you; a substitute is not guaranteed. For a program using a single package price, an individually appropriate omission, substitution, or your decision to decline a component does not by itself reduce that price. The whole-service denial and fulfillment remedies under "Cancellations refunds and shipping" still apply.
Standard Taekit fulfillment, branded containers, educational cards, and included medication replacement benefits use the participating pharmacy designated for your program. You retain prescription choice, lawful transfer, and delivery-remediation rights required by law. Contact the pharmacy if you want to transfer a prescription to a pharmacy outside your program. Other pharmacies may not offer the same kit, packaging, educational resources, benefits, or price; no equivalent-price promise is made. You are responsible for costs charged by a pharmacy you independently select. An elective transfer does not by itself reduce the package price or create a refund right, but the cancellation policy and nonwaivable rights still apply. Your Clinical Provider and dispensing pharmacy will not obstruct a transfer required by law. If Taekit arranges a change to the participating pharmacy, any material effect on fulfillment, benefits, or charges will be explained, with any legally required choices or remedies.
The pharmacist will supply required labels, information and access to counseling. Ask the pharmacist about instructions, interactions, substitution choices, storage, expiration or safe disposal. Do not remove individual prescription labels or rely on an educational card to resolve a dispensing question.
Keep medicines in their labeled containers, secure from children and other people, and follow the storage conditions and use-by date on each product. In general, medications should be stored at room temperature (68-77°F). Certain medications have different storage requirements. Please contact the pharmacy with any medication storage questions. Do not use an expired, damaged, recalled or potentially compromised product; contact the pharmacy. A kit's service end date is not a medicine's expiration date.
Price and service period
For a standard retail purchase, your order summary identifies the single package price, taxes, and initial shipping charge, if any, before payment. The package includes the case, applicable medications, pharmacy services, the initial HHP evaluation, and the included services described below. Taekit collects the stated payment at purchase, before the medical intake and evaluation. Payment does not buy a guaranteed prescribing decision. Once paid, you owe no additional amount for those expressly included items and services; separately disclosed replacement pharmacy charges, shipping, and excluded services may still apply.
The standard retail medical service period, provided by HHP, begins on the pharmacy's initial dispensing date and ends 12 months later. During that period, HHP's clinically appropriate kit-related consultations are included at no additional clinical fee. Clinically appropriate replacement medication after authorized use is unlimited, subject to an individual prescribing decision and lawful dispensing requirements. Any additional pharmacy charges and shipping costs are the patient's responsibility unless a disclosed sponsorship arrangement covers them. Replacement medication does not extend or restart the original service period.
An employer-sponsored or partner-practice program may have different included services, service duration, Clinical Provider, pharmacy, or funding arrangements. Any differences from the standard retail offering will be identified in the program summary made available before purchase or enrollment. That summary identifies what is included and who is responsible for any additional costs. Where the employer funds the full initial kit cost, the employee is not charged the kit price, initial shipping, or taxes. The disclosed terms determine responsibility for later replenishment or other costs. A private sponsor agreement does not silently reduce benefits disclosed to the participant. Unless a different arrangement is expressly disclosed and accepted, the standard retail benefits described here apply.
The care included is limited to the disclosed kit program and does not pay for outside visits, testing, emergency care, or another provider's charges unless expressly included. The program is not health insurance, does not replace health coverage, and does not itself provide comprehensive primary care services.
There is no automatic renewal or recurring subscription charge under these terms. At the end of the disclosed service period, continued participation requires a new purchase or renewed sponsored enrollment and any required clinical assessment. The then-disclosed price and program benefits apply. The service period is distinct from a prescription's legal validity, available refills, medication expiration, and authorization to begin a particular treatment course. Those depend on applicable law, the prescription, product labeling, and clinical instructions. Taekit or the care team may contact you about the service end date and renewal options. You are not obligated to renew.
Cancellations refunds and shipping
To cancel, contact Taekit at support@taekitmedical.com. If we receive your cancellation request before the pharmacy has dispensed your medications, Taekit will refund the complete kit purchase price paid. Likewise, if your Clinical Provider determines that you are clinically ineligible for the entire program, the full kit purchase price paid will be refunded. Refunds follow the actual funding arrangement. Employer-funded amounts are returned or credited to the sponsor rather than paid as cash to an employee who paid nothing. For a gift-card redemption, the refund is restored to the gift-card value or otherwise returned through the original funding method as applicable, subject to any cash-redemption rights required by law. This kit-order cancellation provision does not create an unrestricted cash-refund right for an unredeemed gift card.
After the pharmacy dispenses your medication, there is no change-of-mind return or refund for prescription medication. Do not mail medication back without instructions from the pharmacy. This policy does not limit remedies for an incorrect, undelivered, damaged, recalled or otherwise nonconforming order, legally required cancellation or any other nonwaivable right. Refunds are not denied merely because returned prescription medicine cannot be dispensed to another patient.
The pharmacy ships to the delivery address provided for your order. Tell Taekit promptly if it changes; a change may not be possible after fulfillment has progressed or shipment has occurred. You will receive shipment confirmation and available tracking information by email. Shipment means the package has been sent; it does not mean delivery has occurred.
If shipment is delayed, Taekit and the pharmacy will provide the notices and cancellation/refund options required by law. When no shipment time is stated, the federal merchandise rule generally requires shipment within 30 days after a properly completed order, or a compliant delay-consent and refund process.
If an item becomes unavailable, the clinician may propose a clinically appropriate alternative. A material fulfillment change will be explained, and legally required acceptance or cancellation options will be provided. An unavailable product is not treated as a clinical contraindication simply to avoid a fulfillment remedy.
Report a missing, damaged, incorrect, or temperature-compromised package promptly to the pharmacy; do not use affected medicine while its safety is in question. Taekit support can assist with routing and order coordination. The pharmacy will assess the problem and replace compromised medicine or arrange prompt dispensing through an appropriate pharmacy as required by law. Taekit will contact you in the event that your medication is subject to a recall notice. Follow recall instructions promptly. These remedies are separate from routine replacements after authorized use.
Taekit may receive and transmit information needed to coordinate prescription, refill, or replenishment requests between your Clinical Provider and pharmacy and to track fulfillment and shipment. Taekit may send status updates, expiration or recall notices, and other service notifications on their behalf. The treating professional remains responsible for prescribing and treatment authorization, and the pharmacist remains responsible for dispensing. A request, reminder, or status notification does not guarantee a prescription or authorize medication use.
Payment programs and estimates
Taekit does not bill insurance. The standard HHP program is cash pay, but Medicare, Medicaid, or other applicable program rules may affect whether and how particular services can be charged to you. Tell the care team if you have Medicare, Medicare Advantage, or Medicaid before paying for clinical services. An agreement to self-pay does not waive mandatory billing, coverage, or beneficiary protections. The responsible Clinical Provider will explain a lawful payment arrangement before charging you. Employer sponsorship or a partner-practice arrangement does not waive those protections.
If you qualify as uninsured or self-pay for an item or service, you may request a Good Faith Estimate of expected charges before care. Contact care@taekitmedical.com; the centralized care team will assist with the request and route it to the responsible provider or pharmacy as appropriate. The website's "Clinical Privacy and Patient Notices" page explains estimate and billing-dispute rights. A package price or sponsor payment does not replace an individualized estimate when one is required.
Privacy communications and recording
Your Clinical Provider maintains the clinical record and handles health information under its Notice of Privacy Practices and applicable law. Taekit may receive, maintain, and transmit information needed for authorized program administration and service coordination, including limited prescription or medication information, refill requests, and fulfillment status. Access is limited according to each person's responsibilities. General Taekit order-support personnel do not receive unrestricted access to clinical records. Required confidentiality and business associate arrangements apply when Taekit or other service providers handle protected health information on behalf of a Clinical Provider or pharmacy.
Taekit's Privacy Policy describes its information practices. HHP's Notice of Privacy Practices and related patient notices are available through the website link labeled "Clinical Privacy and Patient Notices." If another practice provides your care, its notice applies to its services; the centralized care team can help you obtain a copy or route questions to that practice. The dispensing pharmacy maintains its own applicable privacy notice. Acknowledging receipt of a clinical privacy notice confirms receipt only. Agreeing to these terms or receiving a notice is not a blanket disclosure or marketing authorization and does not waive privacy rights.
Your Clinical Provider and authorized care-coordination personnel may call, text, or email about care, scheduling, and existing kit services using the contact details you provide, subject to applicable consent requirements and your preferences. Clinical discussions use secure messaging by default. Ordinary text messaging may be available after privacy risks are explained and your choice is documented. Ordinary email and text can be seen by others with access to your device or account and may have privacy or delivery limitations. Identity or authority may be verified before information is disclosed. Tell the care team if your contact details change or you need a different reasonable communication method. Do not use messages for emergencies.
Taekit may send promotional emails about its own kits and services to purchasers and participating patients, including employer-funded participants, where permitted by law. Employer-roster information may be used for benefit invitations and activation reminders; receiving those messages alone does not enroll a person in general promotional emails. We do not send paid third-party promotions or promotional text messages. You may unsubscribe using the email link or support@taekitmedical.com. Necessary order, safety, and service messages may continue.
We keep medication-specific care and fulfillment messages separate from unrelated promotional emails. Information entrusted to Taekit for a Clinical Provider's or pharmacy's services is used for those authorized purposes and is not repurposed for unrelated marketing without any separately required permission.
Clinical recording and AI-assisted documentation. Your Clinical Provider or authorized clinical support personnel may use recording, AI-assisted transcription, and summarization to assist with documentation during nursing and provider encounters. The responsible care team explains this use before recording or AI transcription begins and obtains any consent required by law. You may decline or ask that recording or transcription stop without losing otherwise available care. The responsible healthcare professional reviews AI-generated documentation before relying on it for care and remains responsible for clinical decisions.
Clinical information safeguards. Clinical recordings, transcripts, and notes are maintained by the responsible Clinical Provider and its authorized service providers. Access is limited to authorized clinical and support functions under applicable confidentiality requirements and required agreements. General Taekit order-support personnel do not receive unrestricted access to these materials. These terms do not authorize their use for advertising or unrelated AI model training. Declining or stopping recording or transcription does not require deletion of information already lawfully collected or records that must be retained.
Administrative support and AI assistance. Taekit provides administrative support by email at support@taekitmedical.com and may use AI tools to organize, transcribe where applicable, or summarize support communications for quality assurance, administration, and fulfillment. Providers of those tools are restricted to authorized services and may not use identifiable customer communications to train general-purpose AI models. Where recording or AI transcription is used, Taekit provides advance notice and obtains required consent; you may decline and still receive assistance. Administrative AI does not provide clinical advice. Direct clinical questions to care@taekitmedical.com or the clinical nurse triage line supplied with your kit.
Electronic documents and copies
You may access, save, or print these terms, Taekit's Privacy Policy, and the notices on the "Clinical Privacy and Patient Notices" page through website links. You may request paper copies without charge, a copy of the terms accepted for your purchase, or help accessing documents. Contact support@taekitmedical.com for purchase terms and Taekit privacy information. Contact care@taekitmedical.com for clinical notices and medical-record requests; the centralized care team will assist and route requests to the responsible organization. That organization remains responsible for responding to requests concerning its records or practices. Legally required notices will be provided through a permitted method, with any additional electronic-delivery consent that applies.
Ending service and this agreement
You may stop using the program at any time, subject to the refund provisions above. Your Clinical Provider may decline an unsafe prescription or treatment and may end a clinical relationship when law and professional duties permit, with appropriate notice, transition instructions, and access to records. Ending service does not authorize self-treatment from the kit.
The terms, disclosed program benefits, and price accepted for your purchase or enrollment govern that service period. A later website version does not retroactively reduce those benefits. A material change to those benefits requires your agreement, except where applicable law requires a change; Taekit will explain any resulting cancellation or refund rights. Updating a privacy notice does not itself change purchased benefits or supply consent for a new use or disclosure that requires separate permission. Additional consent or agreement will be obtained when required for the particular change or service. Texas law governs to the extent applicable, without eliminating other mandatory law where care is provided. Nothing in these terms waives professional responsibility, privacy rights, consumer remedies, or other rights that cannot lawfully be waived.
If an employer sponsors your kit, you still make your own treatment and consent choices. Under this program, employers do not receive named participation or fulfillment reports, individual order numbers, medical records, or individual treatment information. Employer reports and invoices use aggregate information and employer-level references, with safeguards against identification in small groups. The Privacy Policy describes these limits. An employer's payment does not authorize access to an employee's medical record. A gift purchaser likewise does not obtain access to the recipient's order or clinical information merely by paying; disclosure requires the recipient's permission or another applicable legal basis.
Severability. If a court with authority over a dispute finds any provision of these terms invalid or unenforceable, that provision will be ineffective only to the extent of that finding. The remaining provisions will continue in effect to the extent permitted by law, unless the affected provision is essential to the agreement.
No waiver. A delay or failure by you or a participating organization to enforce a provision does not, by itself, waive the right to enforce it later. Waiving a particular breach does not waive a different or later breach. This provision does not extend any legal deadline or limit rights that cannot lawfully be waived.
